Abstract

Modern healthcare increasingly involves patients with multiple interacting clinical, diagnostic, therapeutic, psychological, social, and functional needs. The traditional model in which individual professionals work independently is often inadequate for complex patients who require coordinated assessment and continuous management. Interdisciplinary healthcare brings together professionals from different disciplines to combine their expertise around a common patient centered care plan. This article examines an integrated model involving pharmacy, radiology, respiratory therapy, medical laboratory services, nursing, and social work. Each discipline contributes a distinct but interconnected component to the patient-care pathway. Pharmacists promote medication safety, optimize pharmacotherapy, identify drug-related problems, and support adherence. Radiology provides diagnostic information that influences disease detection, staging, treatment planning, and monitoring. Respiratory therapists contribute to airway management, oxygen therapy, pulmonary assessment, ventilatory support, and respiratory rehabilitation. Medical laboratory professionals provide objective biochemical, hematological, microbiological, immunological, and molecular data required for diagnosis and treatment monitoring. Nurses provide continuous bedside assessment, care coordination, medication administration, patient education, advocacy, and early recognition of deterioration. Social workers address psychosocial determinants, financial barriers, family circumstances, discharge planning, community resources, and continuity of care.
