Bioactive Phytochemicals from Medicinal Plants: Emerging Therapeutic Strategies for Chronic Disease Prevention and Management

Abstract

Medicinal plants have served as a primary source of therapeutic agents for centuries and continue to play a pivotal role in the discovery of novel drugs for the prevention and management of chronic diseases. Bioactive phytochemicals, including polyphenols, flavonoids, alkaloids, terpenoids, glycosides, saponins, tannins, and phenolic acids, possess diverse pharmacological properties such as antioxidant, anti-inflammatory, antimicrobial, anticancer, antidiabetic, cardioprotective, hepatoprotective, and neuroprotective activities. Growing scientific evidence suggests that these naturally occurring compounds regulate multiple molecular signaling pathways associated with oxidative stress, chronic inflammation, metabolic dysfunction, apoptosis, and immune modulation, thereby offering promising therapeutic alternatives to conventional synthetic drugs with fewer adverse effects. This review comprehensively examines the phytochemical composition of medicinal plants, mechanisms of action, pharmacological activities, and emerging therapeutic applications in the prevention and management of major chronic diseases, including cardiovascular diseases, diabetes mellitus, cancer, neurodegenerative disorders, obesity, and chronic kidney disease. Furthermore, recent advances in phytochemical extraction, nanoformulation, metabolomics, artificial intelligence-assisted drug discovery, and precision phytomedicine are discussed to highlight future directions in plant-based therapeutics. The review also addresses current challenges related to bioavailability, standardization, toxicity assessment, clinical validation, and regulatory approval. Overall, bioactive phytochemicals represent a promising frontier for developing safe, effective, and sustainable therapeutic strategies for chronic disease prevention and personalized healthcare.
